Living Well with Diabetes: Toolkit

This 6-week self-management class meets by phone weekly for 30 minutes. Participants will be guided through the Toolkit resources and learning objectives. Learn self-management skills such as goal setting, stress management strategies, understanding glucose monitoring, and much more.

Toolkit includes:

* Living Well A Healthy Life textbook

* Exercise CD

* Relaxation CD

* Weekly small group conference call with instructor
